Assisted Reproductive Technologies (ART)

Assisted Reproductive Technology encompasses procedures where eggs and sperm are handled in the laboratory to create embryos. These are the most advanced fertility treatments available.

IVF (In Vitro Fertilization)

IVF is the most well-known fertility treatment and often the most effective for many types of infertility. The process involves stimulating the ovaries to produce multiple eggs, retrieving these eggs, fertilizing them with sperm in the laboratory, and transferring resulting embryos into the uterus. IVF is indicated for tubal factor infertility, moderate to severe endometriosis, low sperm count or motility, and unexplained infertility.

ICSI (Intracytoplasmic Sperm Injection)

ICSI is a specialized form of IVF where a single sperm is injected directly into an egg. This technique is particularly valuable when male factor infertility is present - such as low sperm count, poor motility, or abnormal morphology. It's also used when previous IVF cycles showed poor fertilization. ICSI significantly improves fertilization rates in cases of male factor infertility, often turning hopeless situations into viable pregnancies.

IUI (Intrauterine Insemination)

IUI is a simpler, less invasive procedure compared to IVF. It involves preparing sperm in the laboratory and directly placing it into the uterus around the time of ovulation. IUI is suitable for couples with unexplained infertility, mild male factor infertility, cervical factor infertility, or low ovarian reserve where IVF risks might be high. The advantage of IUI is lower cost, fewer medications, and minimal invasiveness. Dr. Shitole often recommends IUI as a first-line treatment in appropriate cases, reserving IVF for situations where IUI is less likely to succeed.

Surgical Treatments

Many fertility problems are caused by anatomical issues that benefit from surgical correction. Dr. Shitole's advanced surgical training enables him to address these problems using both minimally invasive and robotic techniques.

Diagnostic and Operative Laparoscopy

Laparoscopy is a minimally invasive surgical procedure that allows visualization of the uterus, fallopian tubes, and ovaries. It's both diagnostic and therapeutic. Diagnostically, laparoscopy can identify endometriosis, adhesions, fibroids, ovarian cysts, and tubal damage. Therapeutically, many of these conditions can be treated during the same procedure - endometriosis can be excised or ablated, adhesions can be lysed, fibroids can be removed, and tubal blockages can sometimes be cleared. The advantage of laparoscopy over open surgery is smaller incisions, faster recovery, less pain, and better cosmetic outcomes.

Hysteroscopy

Hysteroscopy involves inserting a thin telescope through the cervix to visualize the inside of the uterus. It can diagnose and treat intrauterine problems including polyps, fibroids (submucosal), septate uterus, asherman's syndrome (intrauterine adhesions), and retained products of conception. For many conditions affecting the uterine cavity, hysteroscopy offers a non-invasive approach to diagnosis and treatment, avoiding the need for open surgery.

Male Infertility Evaluation and Management

Male factor infertility accounts for 40-50% of infertility cases. The clinic provides comprehensive male infertility evaluation including semen analysis, scrotal ultrasound, hormonal evaluation, and referral to a urologist when surgical intervention (varicocele repair, vasectomy reversal) is needed. For men with low sperm parameters, treatments range from medical management to ICSI, and in some cases, retrieval of sperm from the epididymis or testis for couples with azoospermia (no sperm in ejaculate). Female Infertility Assessment and Customized Management

Female infertility may result from ovulatory disorders, tubal factor, endometriosis, poor ovarian reserve, or uterine problems. Fertility Preservation (Egg Freezing and Sperm Banking)

Fertility preservation allows people to safeguard their reproductive potential before it may be compromised by cancer treatment, advancing age, or other medical conditions. The clinic offers egg freezing (oocyte cryopreservation) for women wanting to delay motherhood, cancer patients before chemotherapy, or those with medical conditions affecting ovarian function. Sperm banking is available for men before vasectomy, cancer treatment, or for those with declining semen parameters. Embryo cryopreservation is also available - extra embryos from IVF cycles can be frozen for future use.

High-Risk Pregnancy Management

Pregnancies resulting from fertility treatment sometimes carry specific risks, or couples may have other high-risk factors. Dr. Shitole provides comprehensive management of high-risk pregnancies, including frequent monitoring, specialized ultrasounds, coordination with maternal-fetal medicine specialists if needed, and guidance on activity, diet, and precautions to optimize outcomes.

Advanced Diagnostic Testing

For select cases, the clinic offers advanced testing including PGD (Preimplantation Genetic Diagnosis) to screen for genetic diseases before embryo transfer, and PGS (Preimplantation Genetic Screening) to identify chromosomally normal embryos - particularly valuable for women over 35 or those with recurrent miscarriages.
